Dr Kieran Gilmartin - Clinical Director Fareham & Portchester PCN

Dr Kieran Gilmartin is a GP Partner at Centre Practice (since October 2015)and has been the Clinical Director since the formation of Fareham and Portchester Primary Care Network in July 2019. I previously served in the Royal Navy as a doctor and then a GP for 16 years across many ships (including HMS Campbeltown, Somerset, St Albans & Bulwark) and bases through the UK. I enjoy relaxing at home with my wife and 2 boys and our collection of 3 dogs, a snake and a tortoise.

Louise Pechal - Manager Fareham and Portchester PCN

I joined the Fareham & Portchester PCN in April of 2020 and have had the pleasure of being instrumental and at the heart of the growth and development of the PCN alongside the clinical Director Dr Kieran Gilmartin and Centre Business Manager Sue Crowley and more recently my Administrative support team.  Previously I have worked within the NHS for 11years and prior to joining FPPCN I was an administrator for the SDAS and HVS from another local practice. Providing better care for patients is something I am very passionate about. Before that I was a stay-at-home mum to 4 children for 12 years, working from home as a dressmaker and Florist doing wedding arrangements. I have been a Pilates Instructor and successfully run my own Pilates Business.  In my spare time I enjoy spending time with my family and getting off grid in the wild, taking long walks to stay fit and active, reading, quite times and enjoying the fresh air.

Tracey Tudball - Advanced Nurse Practitioner & Home Visiting Service Team Lead

I qualified as a nurse in 1994, following 5 years in industrial psychology. I have worked in hospital, occupational health, research, and the community.  After being redeployed to a COVID field hospital at the start of the pandemic, I joined primary care in 2020. My passion is being closely involved in direct patient care, as well as helping to develop the service. Since joining the PCN I have been lucky enough to help initiate a health and wellbeing clinic for our local homeless hostel. In my spare time I enjoy walking with my dog, exploring, crafting, and cooking for friends and family.

Valentine Ubaezuonu - MSK First Contact Practitioner

I am an MSK First Contact Physiotherapist with keen interest and specialization in musculoskeletal disorders and Rehabilitation Sciences. I have a master’s degree in advanced Musculoskeletal disorders and Physiotherapy from the Katholieke Universiteit Leuven, Belgium{2022} with over 8 years of Clinical experience helping patients with musculoskeletal disorders recover their function and live pain free. In my spare time you will find me cycling , on the tennis court or seeing a movie in the cinemas.

Emily Frankum - PCN Business Manager and Transformation Lead

Working in the NHS, particularly in my locality, gives me the ability to make a real difference to those in my community and have a greater understanding of how our NHS works and where I can make a difference - this is very rewarding and interesting!

I enjoy exploring the world, in particular learning about different cultures and trying their food.  Regularly, I visit my local gym with the aim to stay physically and mentally fit!  We are lucky to have 2 grandsons, with whom we spend time being worn out at soft play! My favourite something has to be my wedding album - so many wonderful memories with family and friends!

Ann Mitchell - Social Prescriber at Centre Practice Fareham and Portchesr PCN

I previously worked in Nursery Nursing in many pre-school settings, later qualifying as a Social Worker, active for 10 years before a career change, working within a residential care home for a year before, becoming a Social Prescriber in Jan. 2020. I enjoy my role, providing help and support to patients, for any non-medical needs. In my spare time, I enjoy dancing and attending music events. I am blessed to live by the sea and love walks along the seashore.
