Fareham and Portchester PCN has 4 Social Prescriber's - Jennie, Marie, Ann and Karen

Social prescribing can improve your health and wellbeing:

Social Prescribers help you to find out about non-medical support and services to improve your mental and physical heath, sense of wellness and independence. The service is available at all 4 surgeries in the PCN - Centre Practice, Gudgeheath Lane Surgery, Portchester Health Centre and Westlands Medical Centre

Social Prescribers can also look for services in your community such as:

  • Befriending, counselling and support groups
  • Volunteering, training and employment opportunities
  • Accessing specialist services and support
  • Opportunities for education and learning
  • Help to lead a healthier and more active lifestyle
  • Ways to develop your own skills and interests

Ask Reception to book you an appointment - you can self-refer and do not need to see a GP to access this service. The social Prescriber's will then call you to discuss your referral and they may come and see you at home or invite you to come and meet them at the GP practice or arrange to meet you elsewhere.
